Findings:
There is a long soft tissue mass effect in the left mid abdomen with a regional decrease in serosal detail.
The right kidney is slightly enlarged while the left kidney is decreased in size and misshaped.
These findings can be due to a mid abdominal soft tissue mass, most likely of intestinal origin(neoplasia vs foreign body) colic lymphadenopathy, focal peritoneal effusion  (peritonitis, neoplastic effusion)
The small left kidney is consistent with chronic renal changes. Right kidney- possible right renal compensatory hypertrophy vs. hydronephrosis, cystic change, infiltrative disease.

Diagnosis:
pancreatitis with septic peritonitis
